"Episode 210" - Follow the journeys of Jeff, a 44-year-old plastic labs technician from Kingsport, Tennessee, and Tammy, a 29-year-old sales support representative from Omaha, Nebraska, from their "before" phases, then as they undergo their various procedures and, finally, when the end results are revealed to their families and friends. Jeff and Tammy were given a truly Cinderella-like experience, as they were able to change their looks in an effort to transform their lives and destinies and to make their dreams come true. This was all accomplished through the skills of an "Extreme Team," including plastic surgeons, a dermatologist, eye surgeons and cosmetic dentists, with a talented team of hair and makeup artists, stylists and personal trainers.

In "Episode 209," "Extreme Makeover" introduced Jeff, who had lost 200 pounds on his own in only one year with a rigid diet and exercise regimen. Jeff had facial surgery and underwent a series of rigorous medical tests in order to be considered for an extended body tuck, a medical procedure that would remove the large amounts of excess skin remaining on his frame. Jeff suffers life threatening complications during tests which indicate that he's a high risk candidate for this procedure. This setback put his extreme makeover in jeopardy, and made him question his decision to go through with the process.

In part two of his story, the long distance and time away from home and from his wife take their toll on Jeff, driving him to the very edge of self-doubt. Because of a prescribed high protein diet to fight anemia, he suffers another setback when he gains 15 pounds. Through the use of "Extreme Makeover's" first ever video diary, Jeff confesses to the cameras his darkest fears and his most raw and honest emotions. But the high risk procedure is his once-in-a-lifetime chance to live the normal life he's always desired, a dream he desperately wants to see come true. He strives to go the distance both mentally and physically, but will it be enough to overcome the odds stacked against him? Jeff had a brow lift with bone contouring, upper eyelid lift, face and neck lift, rhinoplasty, extended tummy tuck, thigh plasty (removal of extra skin on his inner thighs), brachioplasty (removal of extra skin on his upper arms), Lasik eye surgery, 8 upper veneers and Zoom whitening.
