A woman with pulmonary dyspnea is in desperate need of a liver, and if she doesn't get one, her heart is going to give out. When one finally becomes available in West Virginia, Kate and Dr. Jonas drive down to procure it, but they face racial tensions that delay the organ recovery. Another patient who had long been on the waiting list for a liver returns from Mexico having gotten a transplant through the black market. When the liver fails, she winds up in worse condition than before. Dr. Grant faces an ethical dilemma when he's told the woman cannot get a transplant at St. Jude's because she illegally got one in Mexico. Meanwhile, Kate dips her toes into dating waters when she has dinner with a tech-support guy.
